Two copies the nurses and the clients
The nurses: teaching schedule teaching checklist and review answer key
Shopping List: The sooner the Dr. writes the prescription the sooner they can get their supplies and start going out on passes On the back of the shopping list is a list of pharmacies that participate in needle disposal program
May be your first contact with the client.  If not, some teaching may be done. (Questions about the plan)
May have to give out back pack and appropriate videos.
Get nursing copies out of back pack.  Fax white copy as instructed
Review teaching schedule and checklist.
May need to get them started on food information if a week end and no dietician available Get them started on recording Blood Sugars and Urine results immediately in log book.
Encourage Dr. to get prescription early so that they can start getting supplies and going out on pass (Day 2)
Pt should be testing urine regularly and recording
Client or parent should now be drawing up and giving insulin
RN may need to assist with some parts still
Social Worker:  encourage them to discuss expenses, medical coverage, loss and grieving
Dietician: may need extra tear off sheets from our supplies for meal planning
Should have medic alert bracelet before leaving on any passes out of hospital
Home Glucose Monitoring:
     Help pt set up their own machine. Should now be testing with their machine and ours.
     All calculations from hospital monitor must be used.  Record both on chart
     Extra lancets and test strips in locked cupboard
   
Parents and/or patient should be giving own insulin by now and recording accurately
Review  Kids With Diabetes in Your Care,
Medic alert Bracelet before leaving on any passes is on
PASSES:  Passes may be longer and even over supper at this point if family is comfortable.  Can come back for supper and insulin ( OR) If they have insulin and orders, may stay out for supper and test and give insulin at home.  Must test for sugars at 2000 as well if not back by then.  They are responsible to record for you and report.
Should have emergency kit, Blood glucose Monitor Pouch when on pass from hospital Will need to insure supper insulin is written down for them if on supper pass and that they have insulin with them.
First three items supplied in backpack
Syringes regular or short needle  ultra fine type
Sharps Disposal container (list of participating drug stores on back of shopping list)
Medic Alert Bracelet:  check around some very cheap some very pricey
Emergency Kits.  Several is advisable (kit bags, glove compartment of car, school office, back packs etc)

Check for expiratory date when purchasing
After first teaching session begin each subsequent lesson with the review sheet.
